An 18-year-old inmate at New York City’s notorious penitentiary Rikers Island ended his life on Wednesday, the same day he was supposed to see a psychiatrist, internal jail documents note, according to the Associated Press. The young man, identified as Kenan Davis, was found lying on the floor, AP notes. He had been taken into custody on Friday for violating parole.

A jail representative confirmed the death to AP, stating that an investigation is ongoing and adding that Correction Commissioner Joe Ponte sends his condolences to the boy’s family.

AP notes that Davis’ suicide is the second at the facility since the beginning of the year: Inmate Fabian Cruz, who was supposed to be on suicide watch, hanged himself on New Year’s Day.
