The Grammy Award winner will end her 50-year career with her final studio album, The Dreamer, on Oct. 25. After that, she says, she'll focus on her health.

James has been in and out of the hospital in recent months undergoing treatment for leukemia and has suffered from a potentially deadly sepsis problem, in addition to having been diagnosed with Alzheimerโ€™s disease in 2009.
